Most health insurance providers permit Radiology Ltd. to obtain the pre-authorization for patients referred to our practice.
We will contact the applicable Radiology Benefits Management Program (RBM) for the patient’s insurance carrier. To create a pre-authorization request, please fax the following information:
- Exam Order
- Clinical History (including at minimum the last 3 office visit notes and any labs or other testing)
- Patient Demographics
- Insurance
For STAT order requests, please provide authorization at the time of scheduling to avoid delay in patient care.
