To access your patient’s records, please log in to our Provider’s Portal by clicking on one of the above tabs (RadVision, I-Connect, or PAC’s). If you do not have an account, you can request one by clicking on the “Request Access” tab above.
If you require a CD, please fax your request to our medical records department at 1-520-326-7989 or you can call us Monday thru Friday between the hours of 8:00 am – 5:00 pm at 520-545-1822.
